  2. Email Address
  3. Windows Phone Marketplace Publisher Name
  4. Twitter Id (if you have one)
  5. Link to your latest app
  6. DVLUP user name (sign up here )
  7. Your location, State/Province and City  

You must live in one of the following Provinces or States:

  • British Columbia
  • Saskatchewan
  • Alberta
  • Yukon
  • Northwest Territories
  • Washington
  • Alaska
  • Montana
